Book an appointment to register a birth

Congratulations on the birth of your baby!

We offer pre-booked appointments only, we are not able to accept walk-in appointments. 

Thank you for using the Pembrokeshire Registration Service online booking system.

  • The birth of your baby must be registered within 42 days (6 weeks).
  • If your baby was born more than 42 days ago, call us on 01437 775176.

Please read the following information carefully before completing your booking form.

  • The registration must take place in the registration area where the baby was born. 
    • Babies born at Withybush General Hospital should be registered with us. 
    • If the birth took place at home, please check the location is within the Pembrokeshire Registration District (if you pay your council tax to Pembrokeshire County Council you will be classed as in the Pembrokeshire District).
    • If your baby was born in another district (e.g. Glangwili Hospital, Carmarthen) you can make a 'declaration' at our office and we will send the details to the relevent register office on your behalf. Please note that declaration appointments are only available on Fridays.
  • If you wish to register a birth in accordance with the Human Fertilisation and Embryology Act 2008, or have any questions about same sex birth registrations, please email registrar@pembrokeshire.gov.uk or phone 01437 775176.

Before your appointment

  • Once your appointment has been made you will receive an email and reference number.
    • Please keep hold of the reference number as you will need it if you contact us to amend or cancel your appointment.
    • The email you receive will contain important information about your appointment. Please read the email carefully to make sure you have all the information you need to complete the registration.
  • Use of translators - all informants must be able to understand and speak Welsh or English so they can answer the statutory questions.
    • If you're married or in a Civil Partnership you can translate for each other, if you both choose to attend the appointment.
    • If you're not married or in a Civil Partnership you cannot translate for each other - so you must bring somebody else to the appointment who can translate for you.

On the day of your appointment

  • Please make sure you arrive in good time for your appointment, ideally 10 minutes before the appointment start time so we can confirm your arrival.
  • If you arrive more than 10 minutes late the registrar will not be able to see you and you will need to book another appointment.
  • We can’t allow food or drink into the Registrar’s office (this is in line with Health and Safety requirements)
