Book an appointment to register a death

Thank you for using our online booking system.

  • We offer pre-booked appointments only. We are unable to accept walk-in appointments.

The following information will guide you through how to register a death.

Please read the following information carefully:

A death registration must take place:

  • within five days of the medical examiner issuing the Medical Certificate for the Cause of Death (MCCD)

(The medical examiner will provide you with a reference number and you will need this to book your appointment)

or

  • within seven days of the coroner issuing a CN2 form (a CN2 form is sent from a coroner to your register office to register a death when there is a coroner referral, postmortem, or inquest)

(If the death has been referred to the coroner, you will not receive a reference number, please enter ‘CN2’ when asked for this)

  • In the registration area that the death occurred
    • If the death occurred n a Pembrokeshire hospital or residential care home, it should be registered with the Pembrokeshire register office
    • If the death occurred elsewhere, please double check that the location is within the Pembrokeshire registration district before booking your appointment with us. Find a register office (opens in a new tab)
    • If the death occurred outside of Pembrokeshire, you should either contact the relevant registration district (opens in a new tab) directly to make an appointment or you can make a declaration at the Pembrokeshire register office and we will send the details to the relevant register office on your behalf.

Please note: When choosing the declaration route, it can take a number of days following your appointment to complete the registration and produce the certificates (it can be quicker to go directly to the original registration district). 

Before your appointment

  • Get the reference number for the Medical Certificate Cause of Death (MCCD) from the medical examiner or get confirmation from the coroner (if applicable) that they have issued a CN2 form. 

We will not be able to register the death without one of these forms

  • Once your appointment has been made, make sure you keep a note of your booking reference number in case you need to amend or cancel your appointment.
  • Your confirmation email will contain important information about your appointment, so please read is carefully to make sure you have all the information you need to complete the registration
  • If you are not fluent in English or Welsh you will need to bring someone with you that is able to interpret, there is no restrictions on who can do this for you, it could be a friend/family member etc…

On the day of your appointment

  • Make sure you arrive in plenty of time for your appointment (at least 10 minutes before your scheduled time so we can confirm your arrival.
  • If you arrive more than 10 minutes after your appointment we will not be able to see you and you will need to book another slot.
