Request a death registration appointment

All deaths which occur in England and Wales must be scrutinised by a medical examiner or coroner after which a Medical Certificate Cause of Death (MCCD) or Coroner's Certificate (CN2) will be issued. A death cannot be registered until the Register Office is in receipt of one of these certificates.

Use this form to notify us that you will require an appointment to register a death which occurred in Pembrokeshire. Once the Register Office has received the MCCD or CN2 you will be sent an email with a link to enable you to make an appointment to register the death.
